The problem
Intervention: Receipt of age-appropriate vaccinations according to Ghana's Expanded Programme on Immunisation schedule
Who it was tested with: 306 children aged 0-15 years admitted to Paediatric Emergency Unit at Komfo Anokye Teaching Hospital, Ghana; median age 3.2 years, 55.9% male.
Children or communities are compared with a group that did not receive the intervention, or followed over years. This is the design that best separates what the intervention did from everything else that changed.
"This prospective cohort study collected data on demographic characteristics, clinical diagnoses, outcomes, and immunisation status through caregiver interviews and medical record review using a structured questionnaire" with "logistic regression used to identify factors associated with vaccination default and their outcomes." The study compared clinical outcomes between vaccinated and unvaccinated groups using cohort methodology with statistical adjustment for confounders.
The study is limited to one hospital's emergency unit over three months, so generalisability is uncertain. The multivariate analysis did not find an independent association between vaccination status and mortality, suggesting confounding factors such as disease severity may explain the relationship rather than vaccination alone.
