The problem
Intervention: Prospective measurement of maternal serum C-reactive protein in early gestation and follow-up assessment of adolescent psychotic experiences and academic performance
Who it was tested with: Children from the Northern Finland Birth Cohort 1986, assessed at age 16 years (n=5,071 for psychotic experiences, n=6,770 for academic performance).
Children or communities are compared with a group that did not receive the intervention, or followed over years. This is the design that best separates what the intervention did from everything else that changed.
"Using data from the Northern Finland Birth Cohort 1986 (NFBC1986), a prospective birth cohort including data since before birth, we examined the association between maternal CRP levels in early gestation (N=7,600) and adolescent psychotic experiences (n=396/5,071) and poorer academic performance (n=2,324/6,770), controlling for sex and maternal education level using multivariable regression analysis." The prospective collection of maternal serum before birth and subsequent measurement of offspring neurodevelopmental outcomes provides stronger evidence than retrospective designs.
Observational study cannot prove causation; cannot exclude unmeasured confounding; CRP is a general inflammation marker, not specific to any particular infection; cross-sectional outcomes at single time point
