The problem
Intervention: No specific intervention was tested; this is an observational study examining the associations between early-life factors (growth, SES, maternal education) and child development
Who it was tested with: 636 children from the Birth to Twenty Plus longitudinal South African birth cohort with complete data from pregnancy through age 5 years.
Children or communities are compared with a group that did not receive the intervention, or followed over years. This is the design that best separates what the intervention did from everything else that changed.
"This study used data from the Birth to Twenty Plus study, a longitudinal South African birth cohort study... Multivariable regression analyses were used to investigate the association between SES, maternal education, growth and child development, and structural equation modelling was used to analyse the mediation of growth." This is a prospective longitudinal design with measurement of exposures and outcomes at multiple time points and use of mediation analysis, meeting the criteria for EXPERIMENTAL tier; however, the observational nature and lack of intervention mean it provides evidence about associations rather than causal effects, making the evidence for direct intervention effectiveness INCONCLUSIVE.
The study is observational and cannot establish causation. Selection to the analysis sample (n=636 with complete data) may introduce bias. The findings are specific to one South African birth cohort and may not generalise to other contexts or populations.
