The problem
Intervention: Observational follow-up of naturally occurring prenatal maternal depression; no intervention was delivered
Who it was tested with: 12,058 children from the Northern Finland 1966 Birth Cohort, followed from birth to middle adulthood (over 40 years), stratified by maternal antenatal depression status and parental mental disorder history.
Children or communities are compared with a group that did not receive the intervention, or followed over years. This is the design that best separates what the intervention did from everything else that changed.
"The offspring were followed for over 40 years, and hospitalised severe mental disorders were detected using the Finnish Hospital Discharge Register." This prospective cohort design with 40+ year follow-up and registry-based outcome measurement provides strong longitudinal evidence, though the observational nature means causation cannot be definitively established.
Mental disorders identified only through hospital discharge, capturing severe cases requiring hospitalisation; maternal depression assessed by single self-report question; unmeasured confounders possible despite adjustment for known variables
