The problem
Intervention: Deinfibulation (surgical reversal of infibulation before or during pregnancy/delivery) and comparison of obstetric outcomes in women with type III FGM
Who it was tested with: Women with type III FGM/C (infibulation) delivering across different settings worldwide, compared with women without FGM/C.
Experience from people who deliver programmes: stories, case studies and lessons learned. Valuable for understanding how and why, and for spotting what to test next, but not a measure of effect.
"14 observational studies including 15,320 type III FGM/C women and 59,347 controls were eligible. DerSimonian-Laird random effects meta-analysis was performed and pooled effect estimates with corresponding 95% confidence intervals were computed." This is an observational meta-analysis design comparing exposed and unexposed groups, classified as EXPERIMENTAL because it includes comparative studies with multiple sites and settings, though individual studies were observational. The findings show type III FGM increases adverse outcomes, but deinfibulation mitigates these, indicating MIXED evidence on the overall clinical problem.
The overall level of evidence was low according to GRADE assessment. Substantial heterogeneity was observed across studies (I² up to 96%), multiple analyses were performed increasing risk of spurious findings, and confounding may not have been adequately controlled despite adjustment attempts.
