The problem
Intervention: 4CMenB vaccine (containing fHbp, PorA, NHBA and NadA antigens), available from 2 months of age
Who it was tested with: Clinical meningococcal serogroup B isolates from invasive disease cases in Belgium, 2016-2022; analysis covers all age groups.
Experience from people who deliver programmes: stories, case studies and lessons learned. Valuable for understanding how and why, and for spotting what to test next, but not a measure of effect.
"High-quality WGS data were obtained for 281 of these strains" and "Vaccine coverage was predicted using the Genetic Meningococcal Antigen Typing System (gMATS) and the Meningococcal Deduced Vaccine Antigen Reactivity (MenDeVAR) index". This retrospective genomic surveillance approach examines whether circulating strains match vaccine components, providing evidence that the vaccine targets most but not all strains in circulation.
Study predicts vaccine coverage through genetic analysis without clinical trial data. Cannot confirm actual protection in vaccinated populations. High genetic diversity (23.5% novel antigens) means approximately 20% of strains may escape vaccine-induced immunity.
