The problem
Intervention: Point-of-care C-reactive protein testing device combined with general practitioner education and training
Who it was tested with: Children with acute infections consulting general practitioners across 80 practices in Latvia.
Children or communities are compared with a group that did not receive the intervention, or followed over years. This is the design that best separates what the intervention did from everything else that changed.
"Post hoc analyses of randomised controlled intervention study" with "two GP groups were switched" between intervention and usual care periods. This crossover design with measurement of outcomes before and after intervention assignment, combined with randomisation of GP practices, constitutes an experimental design.
Post hoc analysis may not have addressed all outcomes the original trial was designed to measure. The study does not report clinical outcomes for children or actual reductions in unnecessary antibiotic use. The finding that test results did not guide prescribing suggests the intervention was incomplete.
