The problem
Intervention: Training mothers to use doppler ultrasound probes to monitor fetal heart rates during labour in partnership with maternity staff, with normal range defined as 120-160 beats per minute
Who it was tested with: 6,444 mothers with live fetuses at onset of monitoring attending 4 hospitals in Liberia.
Experience from people who deliver programmes: stories, case studies and lessons learned. Valuable for understanding how and why, and for spotting what to test next, but not a measure of effect.
"Mothers attending 4 hospitals in Liberia consented and were trained to use doppler ultrasound probes to monitor for abnormalities or changes in fetal heart rates during labour...intermittent auscultation was undertaken sequentially, immediately following the end of every uterine contraction." The study measured outcomes prospectively in a defined population receiving the intervention, allowing assessment of whether detection of abnormalities correlated with clinical outcomes and interventions.
No comparison group or control arm to establish causation; unclear whether improvements resulted from maternal monitoring or concurrent improvements in standard care; implementation fidelity not described; selection bias possible as study only included mothers who consented and completed training
