The problem
Intervention: Complex continuum of care intervention delivered by community health workers from preconception through pregnancy, infancy and childhood, including health literacy material, multimicronutrient supplementation, health services and feedback, and behaviour change support sessions targeting nutrition, p
Who it was tested with: Women aged 18-28 years (n=6,800) recruited from Soweto, an urban-poor area of Johannesburg, South Africa, and their offspring followed through age 5 years.
Children or communities are compared with a group that did not receive the intervention, or followed over years. This is the design that best separates what the intervention did from everything else that changed.
"Starting preconception and continuing through pregnancy, infancy and childhood, the intervention is designed to improve nutrition, physical and mental health and health behaviours of South African women to offset obesity-risk (adiposity) in their offspring." The randomised controlled trial design with a defined comparison group and objective measurement of offspring adiposity using dual-energy X-ray absorptiometry provides the potential for rigorous evidence, though results are not yet available.
This is a protocol paper with no results yet reported. The complexity of the multi-component intervention makes it difficult to identify which specific elements drive any observed effects. Long-term follow-up to age 5 years means results will not be available for several years after trial completion.
