The problem
Intervention: Measurement of early-life lung function (tidal breathing patterns) to identify mediation pathway between maternal asthma and offspring respiratory outcomes
Who it was tested with: 1,203 term infants from two birth cohorts (Australian Breathing for Life Trial and Swiss Basel-Bern Infant Lung Development).
Children or communities are compared with a group that did not receive the intervention, or followed over years. This is the design that best separates what the intervention did from everything else that changed.
"Multivariable logistic regression assessed associations, and generalised structural equation modelling was used for mediation analyses." The prospective design with objective measurement of lung function and medical record verification of bronchiolitis hospitalization, combined with longitudinal follow-up, provides evidence that early lung function deficits are associated with respiratory outcomes and partially mediate maternal asthma effects.
Mediation effects were small (3-11%), indicating lung function explains only a minor portion of increased respiratory risk from maternal asthma; other mechanisms remain unexplained. Asthma diagnosis relied on parent report rather than clinical assessment.
