The problem
Intervention: Lifting of non-pharmaceutical interventions (mask-wearing, social distancing, quarantine measures) when COVID-19 was downgraded on May 8, 2023
Who it was tested with: Japanese population covered by JMDC insurance (16,485,812 insured individuals), with particular focus on children aged 0-5 years.
Children or communities are compared with a group that did not receive the intervention, or followed over years. This is the design that best separates what the intervention did from everything else that changed.
"Interrupted time series analyses using seasonal autoregressive integrated moving average models were performed" to compare pre- and post-downgrade periods. The interrupted time series design with a clear intervention point (May 8, 2023) and defined comparison periods qualifies as experimental because it allows assessment of intervention impact on a population level using time-series comparisons.
The study design cannot isolate the effect of NPI removal from other seasonal or behavioral factors that may have changed simultaneously. The lack of mortality increase may reflect survival bias or delayed effects not captured in the study period.
