The problem
Intervention: Comparison of birth outcomes before and during COVID-19 lockdown (observational study of natural experiment)
Who it was tested with: Pregnant women and infants in Soweto, South Africa; 199 mother-infant pairs before pandemic, 201 during pandemic.
Children or communities are compared with a group that did not receive the intervention, or followed over years. This is the design that best separates what the intervention did from everything else that changed.
"Using a case-control design, to compare the prevalence of low birthweight among infants born before and during the pandemic in Soweto, South Africa" and "Infants born before the pandemic and national lockdown were included in the control group, while infants who were in utero and born during the pandemic were included in the case group." This is an experimental design using a natural experiment (lockdown) with comparison groups, allowing causal inference about pandemic effects on birthweight.
Study design observes associations but cannot definitively establish causation. Mechanisms (stress, service disruption, maternal nutrition) are not individually measured. Limited to Soweto context. Does not measure long-term developmental outcomes.
