The problem
Intervention: Antenatal corticosteroid administration prior to planned caesarean birth
Who it was tested with: Women with singleton pregnancies undergoing planned caesarean birth between 36+0 and 38+6 weeks' gestation in South Australia.
Children or communities are compared with a group that did not receive the intervention, or followed over years. This is the design that best separates what the intervention did from everything else that changed.
"Adjusted risk differences (aRD) with 95% CIs were calculated for neonatal and maternal outcomes" and "We used routinely collected electronic maternity and neonatal data." This cohort design with comparison groups between treatment and non-treatment groups allows for estimation of relative effects, placing it in the experimental tier despite being observational rather than randomised.
Retrospective observational design limits causal inference; data from single region may not generalise; unmeasured confounding possible; mechanisms for gestational-age differences unclear
