The problem
Intervention: Modified SIOP PNET 4 protocol comprising cranio-spinal irradiation (54Gy) with vincristine followed by 8 cycles of adjuvant chemotherapy; children under 3 years received modified UKCCSG/SIOP CNS protocol with 2 weekly chemotherapy for 392 days followed by CSI when required
Who it was tested with: 31 children (mean age 7.6 years, range implied under 18) with medulloblastoma treated at a tertiary centre in Kuala Lumpur, Malaysia between 2000-2017.
Experience from people who deliver programmes: stories, case studies and lessons learned. Valuable for understanding how and why, and for spotting what to test next, but not a measure of effect.
"This was a retrospective review of patients from January 2000 to December 2017." The study measures outcomes (survival, complications) in a cohort treated with defined protocols over time, but lacks a comparison group and the high abandonment rate suggests reported survival may overestimate treatment effectiveness for the broader eligible population.
Retrospective design without molecular subtyping; single centre study; high treatment abandonment confounds interpretation of treatment efficacy; no comparison group; unclear denominator for survival calculations; limited detail on causes of abandonment or baseline characteristics predicting outcomes.
