The problem
Intervention: Development and stakeholder evaluation of a family-centered care coordination tool containing problem lists, medication details, care-team contacts, and updatable goal sections
Who it was tested with: Caregivers, primary care providers, and nurse coordinators involved in the care of children with medical complexity.
Experience from people who deliver programmes: stories, case studies and lessons learned. Valuable for understanding how and why, and for spotting what to test next, but not a measure of effect.
"Stakeholders reviewed prototype templates and completed surveys with quantitative and qualitative items, analyzed using descriptive statistics and rapid thematic analysis." The study gathered perceptions and feedback about a proposed tool but did not implement or evaluate its actual impact on care coordination or child outcomes.
This is a stakeholder evaluation study of a prototype tool with no measurement of actual clinical outcomes, caregiver burden, or implementation effectiveness. Results reflect anticipated rather than demonstrated benefits. The pilot study to test these outcomes has not yet been conducted.
