The problem
Intervention: Risk stratification of infants younger than 3 months based on birthweight and length of hospital stay to guide targeted antimicrobial therapy (meropenem or amikacin for high-risk cases)
Who it was tested with: Infants younger than 3 months of age with suspected or confirmed Klebsiella pneumoniae infection in a Malawian hospital; 38 cases and 76 matched controls.
Experience from people who deliver programmes: stories, case studies and lessons learned. Valuable for understanding how and why, and for spotting what to test next, but not a measure of effect.
"Univariate and multivariate logistic regression were performed on mean-centred data to determine risk factors for infection with Kpn." The study used regression analysis to identify which factors independently predicted Klebsiella pneumoniae infection, but did not test an intervention or compare treatment strategies, making it observational research on risk factors rather than evidence of intervention efficacy.
Single-centre study with small sample size (38 cases). Cross-sectional case-control design without intervention testing. No validated risk prediction tool developed. Difficulty in designing simple empiric treatment rules due to overlapping risk factors.
