The problem
Intervention: National Community Health Assistant Program (NCHAP), which provides primary health care through community health assistants to residents living more than 5 km from a health facility, rolled out two districts at a time
Who it was tested with: Women and their children under five in the program catchment area of rural Grand Bassa County, Liberia (22,315 women, 32,743 children).
Children or communities are compared with a group that did not receive the intervention, or followed over years. This is the design that best separates what the intervention did from everything else that changed.
"allowing for a quasi-experimental observational stepped-wedge impact evaluation" - the staggered rollout gives comparison periods and districts, which fits the experimental tier (quasi-experimental). The primary outcome of under-five mortality showed no reduction.
The study is observational and non-randomized, covers one county and only the early implementation period, and the changes in care-seeking and coverage rest on a before/after survey comparison. The authors note that other interventions will be needed to reduce mortality.
