The problem
Intervention: Different atropine eye drop regimens: 0.05% atropine nightly for one year; 0.01% atropine nightly for two years followed by placebo, tapering placebo, or tapering atropine for one year
Who it was tested with: 199 children and adolescents with myopia in Ireland, mean age 13.9 years (60.8% female).
Children or communities are compared with a group that did not receive the intervention, or followed over years. This is the design that best separates what the intervention did from everything else that changed.
"This was an investigator-led, double-masked, randomized clinical trial of different atropine concentrations and regimens" with "observed changes in cycloplegic spherical equivalent refraction and axial length from month 24, or baseline, to month 36." This experimental design with randomization, masking, and measurement of defined outcomes allows causal inference about atropine efficacy for myopia control.
Single-center study in Ireland; relatively small sample size with 73.5% completion rate in group 1; adverse effects limit tolerability; does not assess ultra-long-term outcomes beyond three years
