The problem
Intervention: pediatric type 2 diabetes prevention clinic with endocrinology evaluation, dietician counseling, and metformin prescription (31% of patients)
Who it was tested with: 297 youth referred to clinic with elevated BMI and insulin resistance/prediabetes/early T2D; 63 with follow-up data (mean age 12.5 years, 68% female, 35% Black, 29% White, 30% Hispanic).
The same group is measured before the intervention and again afterwards, with a baseline and an endline and sometimes a midline. Change is visible, but other causes cannot be ruled out.
"Retrospective review of new patients seen at a Type 2 Diabetes Prevention (T2DP) Clinic July 2015 - December 2019...The outcome of interest was categorical HbA1c change between patients' initial and most recent T2DP Clinic visit." The study measured the same group before and after clinic intervention with no comparison group, making it a pre-post design.
Severe attrition with only 21% retention for follow-up visits limits generalizability; relatively short follow-up period; single-center design; no comparison group; unclear impact of metformin or dietician counseling on outcomes.
