The problem
Intervention: Universal varicella vaccination program with two-dose schedule
Who it was tested with: Children born between 2008 and 2017 in Tuscany, Italy, targeted by the universal vaccination program.
Children or communities are compared with a group that did not receive the intervention, or followed over years. This is the design that best separates what the intervention did from everything else that changed.
"We applied the screening method to assess varicella vaccine effectiveness (VE) for the first and the second vaccine dose in the birth cohorts targeted by the UVV. We considered the vaccination coverage (VC) at 24 months and at 5-6 y of age, and we verified the vaccination status of each notified or hospitalized case to identify breakthrough varicella cases (BV)." This represents a quasi-experimental approach comparing vaccinated populations against disease surveillance data over an extended period with consistent methodology.
Study was limited to one Italian region and the pre-pandemic period (2010-2019); breakthrough varicella cases were identified but detailed analysis of risk factors for breakthrough disease was not reported; no data on vaccine adverse events were provided.
