The problem
Intervention: Statins, metformin, or vitamin D (at different doses, alone or combined with omega-3) given to pregnant women, compared with placebo
Who it was tested with: High-risk pregnant women (4,078 across eight randomised trials).
Children or communities are compared with a group that did not receive the intervention, or followed over years. This is the design that best separates what the intervention did from everything else that changed.
"A systematic review and network meta-analysis of randomized controlled trials was conducted." This is a synthesis of randomised trials with placebo comparison groups, so it is experimental, but the authors rate certainty as low and the findings as hypothesis-generating, so the direction is inconclusive.
Sparse networks, limited direct evidence, heterogeneity, partly indirect comparisons and low certainty of evidence. The rankings are exploratory, and the findings should not be read as proof of clinical effectiveness.
