The problem
Intervention: Intravitreal ranibizumab (IVR) injection versus laser photocoagulation (LP)
Who it was tested with: Infants with stage 2 or 3 ROP with plus disease in zone II, randomized at birth; 29 children (10 LP, 19 IVR) followed to school age (~9 years).
Children or communities are compared with a group that did not receive the intervention, or followed over years. This is the design that best separates what the intervention did from everything else that changed.
"In this follow-up of a single-center randomized trial, infants with stage 2 or 3 ROP with plus disease in zone II were randomized in 2014 to bilateral LP (n = 25) or IVR (n = 25). At school age, 29 children (10 LP, 19 IVR) underwent assessment of best-corrected visual acuity, cycloplegic refraction, ocular biometry, anterior segment parameters, spectral-domain optical coherence tomography, optical coherence tomography angiography, and full-field electroretinography." The study compared long-term functional and structural outcomes after the two established treatments for severe ROP.
Small cohort size with differential loss to follow-up reduces statistical power and confidence. Single-center study limits generalizability. Some structural and electrophysiological differences may not have clinical significance.
