Intervention: Ketogenic diet therapies in various forms
Who it was tested with: Children (up to 12 years), adolescents (13+ years), and adults with drug-resistant epilepsy.
Children or communities are compared with a group that did not receive the intervention, or followed over years. This is the design that best separates what the intervention did from everything else that changed.
"Randomized controlled trials (RCT) with a minimum follow-up of 28 days were included. There were 1193 articles retrieved after duplicates were removed and 17 met the inclusion criteria." This is a systematic review of RCTs, meeting the EXPERIMENTAL tier criteria by synthesizing evidence from trials with comparison groups and defined follow-up periods; the overall outcome shows that ketogenic diet therapy works for reducing seizures in drug-resistant epilepsy, though with varying certainty levels across outcomes.
Limited certainty of evidence for higher seizure reduction thresholds (≥90%); scarce and heterogeneous studies on quality of life, cognitive, and behavioral outcomes; inconsistent results on adherence; very low certainty evidence on adult side effects; studies had 6-24 month follow-up periods.
