The problem
Intervention: Cognitive Orientation to Daily Occupational Performance (CO-OP) approach, a cognitive strategy-based, occupation-focussed intervention
Who it was tested with: Children aged 3-18 years with neurodevelopmental disabilities.
Children or communities are compared with a group that did not receive the intervention, or followed over years. This is the design that best separates what the intervention did from everything else that changed.
"In five trials comparing CO-OP with active alternative interventions, meta-analysis did not demonstrate clear superiority for COPM Performance (MD = 1.03, 95% CI [-0.38, 2.44], I2 = 77.7%) or Satisfaction (MD = 1.53, 95% CI [-0.13, 3.19], I2 = 75.1%); certainty was very low." The review uses randomised controlled trials with comparison groups and meta-analytic synthesis, meeting the criteria for experimental evidence tier, though findings are mixed across comparisons.
Very low certainty of evidence when comparing CO-OP to active interventions; high heterogeneity (I2 = 77.7% for performance, 75.1% for satisfaction); no analysis of longer-term outcomes; limited evidence for generalisation beyond individualised goals
