The problem
Intervention: 10-valent pneumococcal conjugate vaccine (PCV-10) given as part of routine immunization program
Who it was tested with: Children aged 6 weeks to 59 months with reported fever (≥38.0°C) and respiratory features consistent with suspected pneumonia in Kenya.
Experience from people who deliver programmes: stories, case studies and lessons learned. Valuable for understanding how and why, and for spotting what to test next, but not a measure of effect.
"We analysed IFBS data collected from 13 sites across Kenya from 2017 to 2024... We assessed the association of vaccination status with hypoxemia... using generalized estimating equations to account for clustering by site." This is an experimental design using real-world surveillance data with adjustment for clustering and multiple confounders across a large population over an extended period.
The study excluded children with confirmed alternative diagnoses, potentially limiting generalizability. The high vaccination coverage may reflect selection bias. The confidence interval for vaccine effectiveness is wide (6-61%), indicating substantial uncertainty in the estimate.
