The problem
Intervention: Clinic-home-community-based nurturing care intervention designed and implemented by mothers2mothers, including activities to promote parent-child interaction and play (such as book-sharing)
Who it was tested with: Pregnant women and their infants born HIV-exposed and uninfected in eSwatini, enrolled between January 2016 and May 2018.
Children or communities are compared with a group that did not receive the intervention, or followed over years. This is the design that best separates what the intervention did from everything else that changed.
"We fitted linear mixed effects regression models with clinic random effects to compare intervention versus comparison arms, and generalised structural equation models to evaluate mediation, adjusting for confounders." This rigorous statistical approach with comparison group allocation by clinic constitutes an experimental design, and mediation analysis identified caregiving activities as the pathway through which the intervention improved language development.
High infant mortality (25 deaths) reduced sample size; baseline imbalances existed between arms for residential location and marital status; lack of effect on maternal depressive symptoms suggests incomplete understanding of mechanisms; follow-up was limited to 18 months.
