The problem
Intervention: Health system strengthening in primary care clinics
Who it was tested with: Children in Ifanadiana district, Madagascar, with analysis of over 1500 households.
Children or communities are compared with a group that did not receive the intervention, or followed over years. This is the design that best separates what the intervention did from everything else that changed.
"We compared changes inside and outside the HSS catchment in the delivery of recommended vaccines, population-level vaccination coverage, geographic and economic inequalities in coverage, and timeliness of vaccination. The impact of HSS was quantified via mixed-effects logistic regressions." This is a quasi-experimental design comparing exposed and unexposed areas over time with regression analysis to isolate the intervention's effect.
The study design does not allow for randomisation of the HSS intervention. The effect of HSS diminished over time and geographic inequalities remained, suggesting the intervention alone was insufficient for universal coverage in remote areas.
