The problem
Intervention: Prenatal exposure to antidepressant medication during pregnancy or up to one month before conception
Who it was tested with: 167,447 children born in Wales, UK between 2009 and 2016, followed until 2022 (mean age 5.68 years).
Children or communities are compared with a group that did not receive the intervention, or followed over years. This is the design that best separates what the intervention did from everything else that changed.
"Generalized estimating equation models with an interaction term between maternal depression and antidepressant exposure were used to investigate SEN and causes of SEN. Predicted probabilities and marginal effects were derived to quantify absolute risk." This prospective cohort design with statistical adjustment allows assessment of associations but cannot definitively separate medication effects from unmeasured severity of maternal depression.
Key limitation is potential confounding by indication and depression severity, as women prescribed antidepressants likely represent a more severely depressed group; depression severity not fully captured in records; unmeasured confounding possible
