Intervention: Assessment of factors associated with successful transition prior to implementing a formal transition program
Who it was tested with: Individuals with sickle cell disease aged 18-24 seen at least twice in pediatric care prior to age 18 at UAB SCD Center in Alabama.
Experience from people who deliver programmes: stories, case studies and lessons learned. Valuable for understanding how and why, and for spotting what to test next, but not a measure of effect.
"We performed a retrospective chart review of individuals with SCD (all genotypes) aged 18-24... Charts were reviewed for demographics, genotype, last known insurance, SCD therapy, clinic location, and transition status. Analyses were undertaken to determine predictors of successful transition." This study identifies baseline barriers and facilitators to transition rather than testing an intervention, making it a practitioner-level assessment of current practice patterns.
This is a single-center retrospective review without a comparison group or intervention being tested. It identifies associations but cannot determine causation or explain mechanisms. The study does not measure outcomes after transition or quality of care received.
